• We are paperless – please complete all online forms before your visit.
• A valid credit card is required for booking to cover no-shows or late cancellations.

Direct Billing

• Available for most major insurance providers.
• Kind reminder, you are responsible for any amount not covered by your insurer.
• If we are unable to bill to your provider for any reason, an e- receipt will be issued to you for your private submission.
